All parents and players must read and sign a “Concussion Information Sheet” in order to participate in the Washington Youth Soccer.  You can confirm that you have read the form on-line at www.skvysa.org.  Go to “Member Log-in”, enter your username and password, and read the form.  In addition, forms are available here:  English Version.  Spanish Version.  For more information email Eric Magnuson

Mmanagers should carry signed copies of the Concussion Information Sheet, along with their signed “consent to treat” forms, to every game and practice.
